Name and age - this is basic information that helps me get to know you and may be used in an RTT session and transformational recording.

Email address and phone number - I use this as a way of contacting you regarding your sessions. I will mainly use the method you first contacted me on but if I cannot reach you, I will try a different method.  

Emergency contact/medical professional details- If I was concerned that you were at risk of harm, I may need to contact your emergency contact or medical professional. I would let you know when/if I am going to do this prior to doing so.  

Session notes - I keep brief notes of our session(s), which are kept confidential in a locked cabinet or password protected document that only I have access to.  

I will not sell or use your data for unethical reasons. I may be required to share your information if my notes are subpoenaed by court, or you or someone else is at risk of harm, in accordance to the code of ethics and my professional responsibility.

Your data will be kept confidential in a locked cabinet or password protected document that only I have access to.  Your phone number(s) may be kept in my business phone contacts with your first name and last initial. Only I will have access to your information.    

I will keep your details and session notes for 7 years as required by my insurer.   After this time I will destroy any document with your personal information and delete your phone number out of my mobile phone.
